Do You Really Need a Referral for Physical Therapy in California?
The short answer is no. Here's what direct access actually means for you.
One of the most common reasons people delay physical therapy is the assumption that they need to see their doctor first, get a referral, wait for approval, and then find a provider. In California, that assumption is outdated — and it's costing people time they don't need to lose.
Here's what you actually need to know about accessing physical therapy in California — and why it matters for getting care at MBODY Physical Therapy and Wellness in Westlake Village.
California Is a Direct Access State
California law allows licensed physical therapists to evaluate and treat patients without a physician referral. You can call MBODY directly, book an appointment, and begin treatment — no prior authorization, no doctor's visit required. This applies to a wide range of conditions: back pain, neck pain, shoulder injuries, vestibular disorders, pelvic floor dysfunction, post-surgical recovery, sports injuries, and more.
Are There Any Exceptions?
A few. California allows direct access treatment for up to 45 days or 12 visits — whichever comes first — before a physical therapist is required to refer you to a physician if you haven't already seen one. In practice, most PT episodes of care fall within that window, and many conditions are fully resolved by then.
Some insurance plans may still require a referral for coverage purposes. This is separate from the legal question of whether you can be seen — a PT can always evaluate you without a referral, but your insurance may reimburse differently depending on your plan. Our team can help you navigate this at the time of booking.
Why This Matters
Time is a real factor in recovery. For acute injuries — a sudden back episode, a sprained ankle, a new onset of vertigo — early intervention consistently produces better outcomes than waiting. The tissue is more responsive, compensatory patterns haven't entrenched, and the nervous system's pain response hasn't amplified.
Requiring a referral visit first adds days or weeks to that window. Direct access eliminates the gap.
For chronic conditions, the calculus is different but the conclusion is similar. If you've been managing low back pain, incontinence, or dizziness for months and haven't yet seen a physical therapist because you're waiting for the right moment or the right referral — that moment is now, and you don't need a referral to create it.
What to Bring to Your First Appointment
Without a physician referral, a first PT visit at MBODY still benefits from:
Relevant imaging reports (MRI, X-ray) if you have them — though imaging is not required to be seen
A list of current medications, particularly if they affect pain, balance, or inflammation
Insurance card and photo ID
A clear description of your symptoms: when they started, what makes them better or worse, what you've already tried
Our therapists will conduct a thorough evaluation and determine whether your condition is appropriate for PT or whether co-management with another provider is warranted. If imaging or further medical evaluation is needed, we'll tell you directly.
